RN Case Manager - Austin, TX


Description

The Registered Nurse Case Manager is responsible for
providing nursing expertise and ensuring all services needed are identified, outlined in the Plan of Care and delivered to hospice patients and families.

Principal Accountabilities

    1. Develops a detailed Plan of Care for each assigned patient in conjunction with the
    Interdisciplinary Group (IDG), patient, family and attending physician.
    2. Assures all services indicated in the Plan of Care are provided.
    3. Participates in IDG as a core team member.
    4. Supervises Home Health Aides/Certified Nurses Aides and Homemaker.
    5. Reports to the attending physician and IDG any significant changes in the
    patient’s condition and requests change of orders and updates Plan of Care as
    needed.
    6. Ensures coordination of care across all settings.
    7. Responsible for pain and symptom control.
    8. Instructs patient and family on techniques of patient care.
    9. Reviews with patient and family the Medicare Hospice benefit and all services
    that are available.
    10. Participates in on-call duties and admissions.
    11. Completes all required documentation according to VistaCare policy, procedure
    and guidelines.
    12. Participates in in-service training for staff and contracted facilities.
    13. Participates in monthly chart audits.
    14. Participates in CQI activities, including monthly documentation review.
    15. Complies with the state and federal regulatory requirements.
    16. Adheres to VistaCare Standards of Conduct.
    17. Has a duty to report any potential misconduct of VistaCare Standards of Conduct
    or federal or state laws and regulations.
    18. Has a responsibility to participate in staff training and education programs.
    19. Administers medications and other treatment in accordance with physician orders,
    generally accepted standards of practice and any federal and state laws pertaining
    to medication administration.
    20. Performs other duties as assigned.

Qualifications

    1. One year of recent medical, surgical, oncology or home care experience.
    2. Registered Nurse with current state license.
    3. Complies with acceptable professional standards and practice.
    4. Knowledge of pain and symptom control related to terminally ill patients.
    5. Demonstrates good communication, negotiation and public relations skills.
    6. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation
    in performing job responsibilities.
    7. Reliable transportation, current driver’s license and current auto insurance.
