Description

The Home Health Aide/Certified Nurse’s Assistant is responsible for providing personal care services for patients and their families. Provides care according to the Certified Nursing Assistant assignment.

Principal Accountabilities

1. Responds promptly to patient, family and caregivers. If unable to perform a
certain task, reports to the Registered Nurse Case Manager or Patient Care
Manager or immediately.
2. Meets safety needs of patient and uses equipment safely and properly (foot stools,
side rails, etc).
3. Gives personal care including baths, back rubs, oral hygiene, shampoos and
changing bed linen as often as necessary.
4. Assists in dressing and undressing patients.
5. Prepares meals.
6. Assists in feeding the patient.
7. Takes and records oral, rectal and axillary temperatures, pulse and respiration
when ordered with appropriate completed/demonstrated skills competency.
8. Turns and positions patients, providing proper care and observation of the skin to
prevent breakdown of tissue over bony prominence.
9. Reports on patient\'s condition and significant changes to the Registered Nurse
Case Manager.
10. Assists in ambulation and range of motion exercise as instructed by the RN Case
Manager.
11. Offers and assists with bedpans and urinals.
12. Performs light housekeeping duties in the immediate living area, including, but
not limited to: vacuuming living area, preparing meals, washing dishes, washing
patient’s laundry, emptying trash, dusting furniture, mopping kitchen and hard
floor areas, cleaning bathrooms and sick-room area.
13. Adheres to VistaCare’s documentation and care policies.
14. Does patient\'s laundry when instructed as part of the Plan of Care.
15. Adheres to VistaCare Standards of Conduct.
16. Has a duty to report any potential misconduct or violation of VistaCare Standards
of Conduct or federal or state laws and regulations.
17. Has a responsibility to participate in staff training and education programs.
18. Performs other duties as assigned.

Qualifications

1. Successfully completed a state-established or other training program and a
competency evaluation program or state licensure program that meet Medicare
requirements.
2. At least eighteen years of age and a high school graduate or equivalent.
3. One year of recent hospice experience or home health preferred.
4. Ability to read and follow instructions and provide documentation of care given.
5. Displays initiative and able to work with minimal direct supervision.
6. Passes criminal background checks.
7. Reliable transportation, current driver’s license and current auto insurance.
8. (Georgia only) Certified Nursing Assistant’s must maintain current CPR
certification.
