DescriptionWhen requested by the patient or family, the Social Worker Principal Accountabilities
the patient or family. 2. Provides short-term supportive counseling services to patients and families. 3. As a member of the IDG, participates in the development of the Plan of Care. 4. Documents all interactions with patients and families according to VistaCare policy, procedures and guidelines. 5. Maintains knowledge of state Medicaid requirements, Advance Directives, POA/DPOA issues and any changes that occur. 6. Assists with discharge planning or transfer arrangements as needed. 7. Develops and maintains a list of community resources and advises patients and families of the availability of community services. 8. Provides referrals to community resources for patients/families. 9. Assists family in completing Medicaid application as necessary. 10. Participates in bereavement activities as requested. 11. Performs on-call duties as required. 12. Participates in Continuous Quality Improvement activities as needed. 13. Adheres to VistaCare Standards of Conduct. 14. Has a duty to report any potential misconduct of VistaCare Standards of Conduct or federal or state laws and regulations. 15. Has a responsibility to participate in staff training and education programs. 16. Performs other duties as assigned. QualificationsOne year experience as a social worker.
