Description

The Spiritual Care Coordinator, when requested by the patient, family, or IDG is responsible for assessing and meeting the spiritual counseling needs of the hospice patient and family.

Principal Accountabilities

Assesses the need for and provides spiritual care services for patients and their families.
As a member of the interdisciplinary team, assists in the development and implementation of the Plan of Care.
Documents all visits with patients/families accurately, completely, within 72 hours.
Interacts with the patients\' own clergy as needed.
Interacts with religious community advising of hospice program and benefits.
Develops and maintains a list of clergy of various denominations who are available to work with our hospice patients/families.
Assists with bereavement support activities as requested.
Performs on-call duties as required.
Other duties as assigned

Qualifications

Graduation from an accredited seminary or other training program, ecclesiastical endorsement, and/or ordination required.
Ability to provide non-denominational spiritual care to patients/families from varied religious backgrounds.
Excellent communication and customer service skills.
Good understanding of dying, death, and the grief process.
Clinical Pastoral Education certification preferred.
One year experience dealing with patients undergoing significant stress, the dying process, or grief preferred.
Previous hospice experience preferred.
