DescriptionThe Triage Nurse position is responsible for screening, prioritizing and resolving problems of patients and caregivers after hours, on weekends and on holidays. The TRN maintains continuity of care for the patient by utilizing the Plan of Care, the attending physician, team physician, on-call staff and supervisory staff. Principal AccountabilitiesResponsible for pain and symptom control. Triages all patient or family calls, and assesses the need for a visit, medication adjustment and/or instructions. Ensures coordination of care across all settings. Responds via telephone to all calls received according to VistaCare guidelines. Communicates with on-call RN when patients or families require a visit. Reports to the attending physician and IDG any significant changes in the patient’s condition, requests change of orders and updates Plan of Care as needed. Contacts pharmacy, Durable Medical Equipment (DME) vendors and others as needed to obtain services. Consults with the on-call administrator prior to a change in the level of care, i.e. transfer to a hospital. Reports deaths, discharges and admissions according to VistaCare guidelines. Informs Patient Care Manager and RN Case Manager of triaged patient status by 8 a.m. the next business day.
QualificationsOne year of recent medical, surgical, oncology or home care experience. Registered Nurse with current state license. Complies with acceptable professional standards and practice. Knowledge of pain and symptom control related to terminally ill patients. Demonstrates good communications, negotiation, and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Reliable transportation, current driver’s license and current auto insurance.
